Medical Report - Certificate of Compliance with the Requirements of Measures Adopted to Prevent the Spread of Infectious Diseases

A certificate of compliance with the requirements of measures adopted to prevent the spread of infectious disease is a document that may be required when you apply for a residence permit in the Czech Republic or for extensions thereof.

If the Ministry of Health has adopted a measure to prevent the spread of infectious disease in line with the Act no. 258/2000 Sb., on the Protection of Public Health, you will be prompted by the Ministry of the Interior as a party to the proceedings to provide a certificate of compliance with the requirements of such a measure.

You can be also requested to provide a certificate of compliance with the requirements of measures adopted to prevent the spread of infectious disease if there are reasonable grounds to believe that you have an infectious disease or that you live in a risk area in the country of origin.

The certificate has a form of a medical report from the content of which it is possible to identify the person for whom it was issued (containing at least data such as the first name, last name, date of birth and nationality), the health centre that issued it (containing its full name, address and the first name, last name and signature of the doctor that treated the patient) and the date of its issuance. It must be evident from the medical report when and where you were treated and it must contain a clear conclusion stating that you do not have an infectious disease for which the measure has been adopted.
