Change of Travel Document Regarding EU Citizens and Their Family Members

REGISTRATION CERTIFICATE OF EU CITIZENS

All EU citizens holding a registration certificate must report a change of a travel document or an identity document.


What is the time limit to report the change?

You must report a change of a travel document within 15 working days. Report the change only after it has taken place.


How and where can you report the change?

You can report the change personally at a Ministry of the Interior office, send it by post, Data Box or by e-mail with an advanced electronic signature, or you can submit it through an authorised representative.


What is the administrative fee?

There is no administrative fee for reporting a change of a travel document.

You can fill in the application form online. Then print out the filled-in form and sign it.

You can also download a blank application form, print it out and fill it in by hand, or you can collect the form free of charge at one of the offices of the Ministry of the Interior. The form must be filled in legibly, in block capitals and in Latin characters.

3. Send the form or make an appointment at a Ministry of the Interior office

You can report the change personally at a Ministry of the Interior office, send it by post, Data Box or by e-mail with an advanced electronic signature, or you can submit it through an authorised representative.

In the case of a civil status change, you do not need to go to a Ministry of the Interior office in person. You can simply report the change by post. If you report the change by post, never send your original travel document – only attach a copy.

If you want to report the change by post, you can check whether it has been registered on our hotline. In this case, it is recommended to call the hotline no sooner than a week after the form was sent as it takes a few days to process the incoming post.

If you wish to make the notification in person, please make an appointment online or by phone beforehand. If the offered appointment slot is after your time limit for notification of the change expires, send the form by post, Data Box or by e-mail with an advanced electronic signature, or visit a Ministry of the Interior office during opening hours without an appointment. With no appointment reservation, you have to wait your turn. If you fail to report the change within the time limit, you can be fined.


PERMANENT RESIDENCE PERMIT OF EU CITIZENS

4. Go to a Ministry of the Interior office

Come to a Ministry of the Interior office to receive your new travel document into your permanent residence permit document. Your permanent residence permit document must contain up-to-date data.

TEMPORARY RESIDENCE PERMIT OF EU CITIZENS’ FAMILY MEMBERS

All holders of a temporary residence permit or a residence card who are EU citizens’ family members must report a change of a travel document or an identity document. If you are a family member of a citizen of the United Kingdom of Great Britain and Northern Ireland under the Brexit withdrawal agreement, the same conditions apply to you.
